We know how difficult it can be to call and schedule a counseling appointment, much less actually go to the first counseling session.
So, we want to assure you that at Daily Bread Counseling we will take the time and effort to welcome you.
Below are the two main forms that will be needed to download and complete. Please bring them with you to your first session.
You will also want to download, read, and then sign one of the following three Informed Consent Forms (please only go to the form, listed below for the counselor that you are seeing) and bring the form with you to your session.
All Counselors who are associated with Daily Bread Counseling are contracted and are not employees of Daily Bread Counseling. You can request a Counselor, however, location, availability and payer source will often determine which Counselor is assigned to you.
Daily Bread Counseling offers a wide range of services, from consultations with Christian family counselors to individuals, relationship and clergy counseling.
